Join Our Caring Community: Become a Shared Living Provider (SLP) in Southern Vermont!

Location: Windsor and Windham Counties Vermont

Health Care and Rehabilitation Services (HCRS) is looking for a compassionate and dedicated individual to join our team as a Shared Living Provider (SLP). In this rewarding role, you'll provide vital care and support for a 19-year-old male with Ehlers-Danlos Syndrome who would thrive best in a routine-based environment and with somebody who has clear communication and a go with the flow personality but is also outgoing and willing to take him into the community for activities. This individual has a pet bearded dragon that will need to remain with him.

Key Responsibilities:

  • Medication Reminders
  • Transportation to and from all appointments and activities
  • Financial support/budgeting
  • Foster a loving, comfortable home environment.
  • Assist with personal hygiene reminders and encourage independence where possible.
  • Work collaboratively with a team of healthcare professionals, including case management, to ensure the best care and support.

About the Individual:

  • Likes: This young man has a love for Halloween and building haunted houses. He enjoys editing films and photography and building/creating crafts. He is a very creative individual. Although needing some assistance with independence this participant finds joy in ordering for himself when out to eat as he is always willing to learn.
  • Support Needs: This individual requires assistance with independence. Although always willing to learn, he does still require guidance. He is wanting to gather skills to become more independent and hopefully get a job which may require transportation. Detailed instructions and communication are a must as this participant needs time to process and adjust to change. He tends to get very anxious when his day doesn’t go as planned and requires time to process and adjust. He will get depressed at times and tend to isolate himself. Personal hygiene reminders are needed and will require support during meal prep and supervision when using the stove.

  • Generous Compensation: A tax-free stipend of $30,000.00 to $45,000.00 per year  plus a monthly room and board payment of $814.65  directly from the individual.
