Your Specific Responsibilities:

This position, an employee of the Nassau County Health Department (NCHD), works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts.   • This position will work to supervise, coordinate, and provide direction to the Community Health Education based programs within the Florida Department of Health in Nassau County. The focus is management of Community Health promotion programs, The Supplemental Nutrition Program for Women, Infants and Children (WIC program), Healthy Start (maternal and infant health care), School Health services and county CHA/ CHIP. Responsibilities of this position include communicating, motivating, training, and evaluating employees; planning and directing their work.

 

  • Provides oversight to community engagement team and ensures coordination and implementation of community outreach programs and initiatives. Management the development of strategies to engage community groups and stakeholders. Ensures oversight of organized community meetings, workshops, and participation in community events. Participates in community and regional coalitions and collaboratives representing local public health.

 

  • Serves as the Community Health Needs Assessment (CHA) and Community Health Improvement Plan (CHIP) Liaison for DOH-Nassau. Leads and supports health improvement projects and initiatives identified in comprehensive CHA report. Provides project management and expertise to the CHIP steering committee with regular meetings, documentation of activities, and progress performance in accordance with the PHAB model. Works in partnership with internal and external stakeholders to develop and implement community health improvement strategies based on in-depth knowledge of evidence-based practices.

 

  • Supervises oversight of the Nassau-Baker County WIC Program to ensure efficiency of time and resources. Evaluates The Nutrition Program Director, by establishing performance standards and presenting performance evaluations according to DOH policy. Supervises program planning, development, initiation, and evaluation of the WIC Program. Ensures programmatic budget is spent accurately and efficiently, including maintaining budget revenues and expenses and purchasing according to FDOH policy.

 

  • Supervises Contractual oversight of Healthy Start program to include supervision of Program Manager and involvement with Healthy Start Coalition. Assurance of monthly deliverables, accurate management of program budget, and assurance of Quality Improvement measures per the annual contract.

 

  • Provides supervisory oversight of the School Health Program for DOH Nassau County. Assures program report deadlines and other deliverables are met or delegated as appropriate. Plans, promotes and implements school health programs in accordance with Florida Statues and DOH administrative rules.

 

  • Responsible for timely entry of EARS, completion of programmatic and state trainings, response during a disaster, compliance with HIPAA and all FDOH Policies and Procedures.

 

  • Performs all other duties assigned. This position is based in one of six Nassau County Health Department Clinic locations

Qualifications:

Minimum-

  • Must have a bachelor’s degree in public health, Health Administration, or Community Health from a U.S. accredited college or university.
  • Minimum of 2 years of professional experience in public health.
  • Must have supervisory experience (documented on resume or application).

Preferred-

  • Must have a master’s degree in public health or health administration, or Community Health from a U.S. accredited college or university.
  • Licensed Dietitian/Nutritionist in the State of Florida
