Job Description
Only Angels Home Health and Hospice, LLC in Wichita, KS is looking for a RN to join our team. We are located on 1624 S Rutan Ave. Our ideal candidate is a dedicated, self-starter with both home health and hospice experience.
Responsibilities
- Evaluate the effectiveness of nursing service, to the patient and family, on an ongoing basis.
- Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
- Prepare, and present, patient's record to the Clinical Record Review Committee, as indicated.
- Consult with the attending physician, concerning alterations of Patient Care Plans; checks with the appropriate supervisor and makes changes, as appropriate.
- Make frequent visits to the patient to evaluate their condition
- Coordinate patient services
- Submit clinical noes, no less often than weekly, and progress notes and other clinical record forms, outlining the services rendered, as indicated
- Provide support and comfort according to individual patient’s needs
- Work with caregivers to plan and execute excellent patient care
- Hospice: Alleviate sudden symptoms of the disease’s end phase
- Willing to take On-Call weekends
- Submit a tally of patient care visits made each day

Requirements and Skills
- Proven experience as hospice nurse/ registered nurse
- Knowledge of nursing care processes and medication guidelines/side-effects
- Committed to safety precautions and high ethical standards
- Sound judgement and attention to detail
- Excellent communication skills and a compassionate nature
- Current CPR certificate
Job Conditions/ Physical Requirements
- Must have a driver's license and be willing and able to drive to patient's residences
- The ability to access patients' homes which may not be routinely wheelchair accessible is required.
- Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care
- Physical activities will include walking, sitting, stooping and standing and minimal to maximum lifting/ turning of patients'
- Must be able to lift up to 50 lbs.
- The ability to communicate both verbally and in writing is required as frequent communication by telephone and in writing in English is required
Qualifications
- Must be a graduate from an accredited School of Nursing (and provide documentation)
- Must be licensed, in the state, as a Registered Nurse (and provide evidence of licensure)
- Home health and/or hospice (preferred, will train right candidate)
- Must have knowledge of Medicare and Medicaid guidelines
- Must submit to a criminal background check
- Must have a current CPR certification (online certification is not accepted)
