Key Responsibilities: • Complete and coordinate all Minimum Data Set (MDS) assessments accurately and on time.
• Ensure compliance with federal and state regulations, including CMS guidelines.
• Collaborate with nursing staff, interdisciplinary teams, and leadership to drive quality improvement initiatives.
• Support care planning and ensure resident-centered documentation.
• Train and guide staff on MDS processes and best practices.
Qualifications: • Current RN or LPN license in Illinois.
• Strong knowledge of MDS assessments, coding, and regulatory requirements.
• Excellent communication, organization, and teamwork skills.
• Passion for resident-centered care and improving clinical outcomes.
