Job Title: Community Health Worker

Pay Rate: $20/hr (Paid Weekly)
Work Location: Remote/Telecommute (Must reside in New Mexico; servicing Southeast, Southwest, Northeast, Northwest, and Metro regions)
Contract: 01/26/2026 to 05/29/2026
Work Schedule: Monday–Friday, 8:00 AM–5:00 PM
Occasional overtime may be required based on business needs.
Position Summary
The Community Health Worker (CHW) supports care coordination teams by locating and engaging hard-to-reach members. This role plays a critical part in member outreach efforts, including visiting members in community settings such as homes, hospitals, and shelters, while promoting access to healthcare services and community resources.
Team Environment
This position works collaboratively with a team of 15–16 Care Coordinators and reports to one manager.
Key Responsibilities
Locate members who are difficult to reach or engage through community-based outreach
Conduct Health Risk Assessments with members
Engage and build rapport with UHC members to support care coordination efforts
Educate members on available Value-Added Services
Collaborate with community organizations and local resources to support member needs
Document outreach efforts and member interactions accurately and timely
Travel within assigned regions as required to meet members in community settings
Required Qualifications
Ability to work independently with minimal supervision
Strong understanding of community culture and local resources
Ability and willingness to travel within assigned regions
Must reside in New Mexico
Valid driver’s license
Preferred Qualifications
Experience working with a Managed Care Organization
Background in case management
Community Health Worker (CHW) or peer support experience or training
Knowledge of Behavioral Health and/or Substance Use services
Licenses & Certifications
No professional license required
CHW or Community Health Representative (CHR) certification preferred
