Job Description

Overview:

Implements pharmaceutical services to patients throughout the regional area Medical Centers by providing accurately compounded and dispensed medications and various clinical functions, including, but not limited to, therapeutic drug monitoring and patient appropriate dosing.

Responsibilities: ACCEPTS RESPONSIBILITY FOR CHECKING ALL MEDICATIONS LEAVING THE DEPARTMENT TO VERIFY QUALITY ASSURANCE.
The pharmacist must have a working knowledge of, or be able to retrieve information about chemotherapy agents.
The pharmacist must have a working knowledge of hyperalimentation.
The pharmacist must have a working knowledge of IV admixture.
The pharmacist must have a working knowledge of narcotic control.
The pharmacist must have a working knowledge of investigational drugs.
The pharmacist must have a working knowledge ofe therapeutic drug monitoring.
The pharmacist must have a working knowledge of medication storage and dispensing guidelines.
DISPENSES MEDICATIONS FROM PHYSICIAN ORDERS.
Ensuring accurate entry of the order into the computer to furnish hospital personnel with current medication administration reports.
Accepting responsibility for verifying dosages on the physician order are patient appropriate.
The pharmacist dispenses medications per policy and procedure in a timely manner, using reference materials when necessary to ensure the accuracy and appropriateness of patient care. The pharmacist is responsible for accurately checking refills, unit dose medications, and IV preparations as to filling and labeling accuracy by the pharmacy technician.
COMPETENT IN COMMUNICATING TO HEALTHCARE PROFESSIONALS REGARDING AN ORDER (I.E., NON-FORMULARY DRUG, ALLERGY; DRUG INTERACTION, DRUG COMPATIBILITIES, DOSING QUESTION, ETC.). MUST BE ABLE TO USE SOUND JUDGEMENT, AS RELATED TO PATIENT CARE, IF THE PHYSICIAN IS UNAVAILABLE TO ANSWER QUESTIONS.
The pharmacist works with healthcare professionals to provide the most appropriate and cost-effective drug therapy for each individual patient. The pharmacist will use the various computer systems and reference books available in the pharmacy to assist healthcare professionals in selecting the most appropriate therapy for the patient.
The pharmacist communicates with healthcare professionals regarding medication orders. Problems or questions with an order may be discussed with either the nurse or with the physician who gave the order. Some of these contacts contain a public relations element while others facilitate good intradepartmental and interdepartmental communication.
The pharmacist plays an active role in planning new functions and updating current services provided by pharmacy to assure high standards of pharmacy practice in the Medical Center.
The pharmacist participates in in-service programs relating to new procedures, policies, equipment and treatments.
The pharmacist participates in projects in the department, or outside the department, which may be formally assigned or may develop informally as the need arises or problems develop.
COMPETENT IN WORKING WITH VARIOUS COMPUTER SYSTEMS, WRITING IN PATIENT CHARTS, DOSING MEDICATIONS BASED ON PATIENT SPECIFIC PARAMETERS, AND OTHER CLINICAL FUNCTIONS THAT MAY ARISE.
The pharmacist enters oral medications into HBOC System.
The pharmacist enters IV medications into HBOC System.
The pharmacist oversees technician in operating Omnicell System.
The pharmacist is able to dose all drugs in Pharmacokinetic Dosing Service.

Salary Range: The compensation range for this position is $51.41per hour - $88.43per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
